Product choosing form

    I am trying to make an almost exact replica of the form in the link below. And i have been trying to find a solution, but most “checkboxes” are not like this.
    And in this form you must start from top and checkbox

    I was informed that he hired someone from India to make this in code, but i was wondering if its possible to use a plugin or something to make this?
