    HI there,

    Thanks for your purchase! Here’s what I can tell you:

    1) Regarding the Authorize Gateway, this can happen if your form was saved with a disabled Gateway as the default. Simply go into your form and make sure that Authorize is selected as the default gateway.

    2) Regarding those additional fields, it seems you added them with Form Field Manager. The form requires the ones toward the bottom, so it’s best if you simply remove the additional fields from Form Field Manager. Check on the select field in the bottom right of the Form Field Manager section to choose a different area of the form to add your fields to so that the default First/Last Name and Email fields can stay at the top of the form.
