Problem with seo plugin conflict

Today I’ve installed WordPress Seo by Yoast plugin and Custom Contact Form has reported some issues that disappears if I deactivate the seo plugin. I cannot see the form I’ve created nor the pages, but the forms are still working sending e-mail notification.
