Problem with form duplication

Hello, I have a issue with form duplication.
When I duplicate a form, the only thing that make is to change the original form adding “_copy” to the name. So it doesn’t create the copied form.
Before I knew this problem I lost some forms…
Anybody has experienced the same issue? I don’t know if this is due a plugin incompatibility. By the way, my installed plugins:
