    Hi there @elnatanrodrigues93

    I see you have Hummingbird active on the site, and the domain is routing through CloudFlare. If the form appearance changes with any plugin activation/deactivation, that does indeed sound like the issue is one or more of these:

    – Page Cache needs flushing
    – Run Re-Check Files in Asset Optimization and its clear cache
    – CloudFlare cache needs flushing

    If you have integrated CloudFlare with Hummingbird on your site, you can flush CloudFlare cache right in Hummingbird.

    But I suspect this is due to Asset Optimization cache.

    Please run the Re-Check Files feature in Hummingbird > Asset Optimization, then clear all caches. Your forms should snap back into shape.
