Problem with checkbox and radio button

Hello,

I just installed the free plugin for test in our staging environment and we noticed the following:
– if we chose a checkbox as a field, the label is displayed, but the actual checkbox is not displayed, and as a result it cannot be clicked
– if we choose a radio button, it is displayed, upon the first click it is checked, but once it is clicked to indicate “gift wrap”, it cannot be unchecked: if you click again on the radio button, it remains in clicked/marked/checked state, so it remains permanently indicating “gift wrap”.

Please advise:
– how can we display the check box
– how the radio button can be consistently checked/unchecked.
