Problem in custom / front-end login page

Hi,

I am using paid version of “Mini orange two factor” plugin.It’s working fine but i have one problem. I have a custom / front-end login page on my site but this custom login page is not working after activating this plugin.

I have also review in your FAQ and find there to “copy shortcode from Advanced Options Tab and embed in login form”. But in my condition no any advance tab is available, there is one premium tab and there is no any shortcode.there is only written “We will provide you the shortcode and setup the end to end flow of 2nd factor in your customized login form”.

So can you please help me to find shortcode or any other solution of my issue.
