problem clicking signup button

I tried signup up a test email(valid email) but when I click “Subscribe”, it takes me to a page where I need to input again my email and name. Please help me I really love this plugin. I thought when I click the subscribe button, it redirects me to thank you page but it doesn’t.

BTW, I’m using MailChimp
