Problem adding form to page.

Hi, I am having trouble adding my form to a page on my site. I have created a form and it is showing under My Forms as 20 Bite Size Foods Quote with Edit, Delete and List icons beside it. As far as I understand the next step would be to create a new page, call it Get a Quote and then there should say FormBuilder below the text box which you use to check the form you would like to insert? However, there is no sign of the box or “FormBuilder under my text box. The plugin is definitely activated as well.
Can someone please help me with this?
