Private reporting or statements app

Hello. I’m looking for a way to add a simple application to our WP site. The functionality is quite simple: The user inputs a predefined ID number and then is redirected to a dynamically generated page where he can print a report of his cash balance in the company’s internal fund.

Is not necessary for the user to be a subscriber, any external DB tools or CRUD frameworks are welcomed too. Anyone have any experience in this? Thanks
