Printable Form

  • I would like to offer printable certificates on my site. The idea is to enable users to add their name using some sort of a form and then have their name appear on the certificate which they can then print in PDF format. I would need to design the form and have the name appear somewhere on it.
    Does anyone know of a plugin that I can use to achieve this?
