Printable Custom “Certificate”

I would like to enable my users to enter their names and addresses on one of my pages and then be taken to a page where 3 “certificates” (3 to a portrait-oriented letter sized piece of paper) are each populated with their specific information and they can print them for their own use.

I may be dense, but I can’t seem to figure out how to do this.

I’m thinking that the certificate would be in PDF format?
