Print Friendly button not displayed on pages with google maps

I have installed the plugin and so far everything is working well except for a few pages where it is not showing up at all. We have a custom template page, so I’m assuming that our template is missing some key bit of wp code that your plugin attaches to for display.
The pages in question are:
Displayed correctly:
