Previous Entries Randomly Replicating

I have a strange situation where someone submits a form on one day and the exact same contents in the form is automatically generated on a different day from the same user name, same ip address, same form name – just a different date/time stamp on a different entry id. The replication can occur the next day or several days later. And it doesn’t necessarily replicate just once. I have one user’s form that has replicated 7 times from the original entry from dates ranging from December 14 (original entry) to January 9. However, it doesn’t happen to everyone else using the exact same form. (That is, I have entries using the same form that have only 1 entry.)
An email is sent out to us each time a new entry id is created, but I don’t know if it is being sent to the originator so I am trying to test if it is being sent out to them too by completing the form myself (in hopes that my entry might just trigger it to randomly replicate.)
Currently using WordPress 5.8.3 and Visual Form Builder Version 3.0.6.
Any suggestions what might be causing the whole form to replicate in the Visual Builder Entries section and sending out at least an email to us?
