Did you end up emailing support? We squash these little issues all the time!

Almost always this issue is caused by a conflict between HappyForms and your theme or another plugin. You can suss this out by disabling those other plugins and see if the preview works.

Once we know which plugin or theme is causing the problem, we can take it from there ??
