If you have made changes to the form and you can see them in preview, then if you have also updated the form they must be applied on front as well.
Note please: you have to click “Update” button, otherwise these changes are not saved and applied.
But if that’s done and they are still not visible, there must still be some sort of cache involved in it. You didn’t attach any link to the page in question so I couldn’t look it up but are you sure these two caches that you cleared are the only ones?
There’s often some sort of server-level cache and/or CDN like CloudFlare or similar and those can also be involved. Furthermore, if you are using any kind of “asset optimization” tool on site (like our own free Hummingbird or any other plugin that can optimize CSS/JS delivery) – it may be necessary to regenerate/refresh assets there too.
