I have the same problem. The plugin works fine except this.

The option is:

MESSAGE UNDER THE LID? *Write a personal message on the bottom side of the lid

If you select ‘Yes’ the textarea box should appear to enter text, but it does not appear.

It does add ￡20 to the product table below, but the text box doesn’t appear in which to add your message.

can you please make sure you have disabled the legacy rendering option from WooCommerce->Settings->PPOM Settings under General tab.

After enabling the legacy inputs rendering, the conditions are not working in the latest version of the plugin. Please see the link to the screenshot below for your reference.
