Post Types with upload image form

Hello everyone

I tryed to assemble all indication on this article

to getting works a new post type with image upload feature.

I’m testing this on wp 2.9 but I always get an error during the activation of the plugin.

Someone could help to test the code and help me to obtain a working version?
