Popup Maker Not Working

2. The Popup should come up with a text and a Gravity Form.
3. When clicked, the overlay comes up but there is not popup box. All methods of closing the popup don’t work.
4. Under the headline of “Request An Appointment” is the button “Schedule Your Appointment Now!”
