Popup Contact form not Generating Lead in Salesforce – no lead generated

I’ve installed this plugin and it’s working fine – displays web form, submits fine, generates lead in Salesforce, all is working great! Thanks for all of your hard work!

I’m contacting you because we’d like to use a popup to display a form on our training page. I’ve configured the popup and it’s displaying properly, the form submits fine and we get an email notification from the form so it’s working well as a contact form – however, it’s not generating a lead in Salesforce?

Do you happen to know how I can get it to generate a lead in Salesforce? Or if you have another WordPress popup plugin suggestion that is compatible with BWL I would be happy to install that one.
