Pop up plugin that links to image

I thought this would be a basic plugin but I can’t seem to find what I’m looking for.

I’m trying to track the number of phone call enquiries I receive via a webpage and have added a telephone image that when clicked will bring up a popup message where I can record the tracking details.

Any ideas will be appreciated….
