Poll with User Details

Hey Guys,

I’m after a plugin that can run a poll, but also allow users to enter their name, email, posting address and phone number.

I need to be able to see the poll results as they are coming in, through the admin area preferably as opposed to publicly viewable.

I’ve used Poll Daddy in the past, and know you can modify the code to allow users to enter details, but was wondering if there is a plugin that would do it as a package without code hacking.
