[Plugin: WP Survey And Quiz Tool] Next Field Button not reacting on custom form

Also, when adding multiple choice answers, the I added one answer “yes”, but when I click on the ADD NEW ANSWER button, it jumps to top of the page, but does not create a fields for me to add a new answer.
