There is a setting in Yoast’s WordPress SEO plugin that breaks this plugin. If you go to the SEO settings > Permalinks in your Dashboard, you will see a checkbox that says “Redirect ugly URL’s to clean permalinks” and a long warning about why not to do so. I had checked that box months ago so it took me a while to figure out that this was the reason Document Revisions was not working on nearly identical WordPress setups.

  • Thanks for the heads up and sorry to hear you’re having trouble.

The problem stemmed from the fact that WordPress would stick a trailing slash on the request prior to serving the document, which WP SEO would then remove, creating an infinite loop. The documents have fake file extensions at the end, which really shouldn’t have a trailing slash, but WordPress didn’t know that.

Got WordPress to stop adding the trailing slash, and the development version should fix the problem. Look for an updated stable release sometime soon.
