[Plugin: WP Customer Reviews] How do I place new fields next to name

Great plugin!

I would like to add the fields Occupation and Location, but when I do so, it puts these fields under the name and date.

What I would like to do is:

1) Not show the field name (like all the other fields in the plugin) by default

2) Display it like this: Name, Occupation, Location. For example, it might look like:

Rating, Jan 3, 2012 by Laurie, Mother of 3, Berkeley, CA
