[Plugin: WordPress Simple Survey] Admin Form submission duplicates fields

Hi,

When we try to configure the survey on the admin form and submit, nothing gets saved and the forms appear in duplicate. So before submission, “Require Info to Submit a Quiz” had 3 fields and after submissions it had 6. The same with all the other fields on the page. They seem to increase / double when the form is submitted and nothing gets saved.
