[Plugin: WordPress SEO by Yoast] Using post_meta data in titles and descriptions

Is there any way to use information in a post_meta field in the templates for the title, description, and keywords? I have a custom post type for businesses and would like to be able to pull in the city and state from post meta into the page title and meta description. Thanks.
