    and sorry for not checking to close the a tag.. I used the link button and pasted my link, but looks like it didnt have a closing tag. This is my second thread, and something went wrong on each of them. :\

    Its okay – WP should be filtering posts for things that can break the layout anyway…

    The filenames only have access to %filename% and the values from a php date format. To do what you need, the form items would have to have access to each other’s data, which would take a lot of rewriting.

    You could use the .csv file of the submission data to cross reference firstname/lastname with the uploaded file name in a spreadsheet program.

    Thanks hoffcamp, I will try figure something out. Great Plugin by the way. How about the custom folder option. Can I have a separate folder inside the wordpress upload folder that I can send all the files to. like %doc_root%/wp-content/uploads/Folder with folder being the new destination
