    I changed the name of the main file from formmanager.php to wordpress-form-manager.php, since that is the correct way to name the plugin main file. To the second problem, do you have the Members plugin installed? The new version integrates with it, so you will have to add permissions to your role if you have that plugin installed.

    I dont have members plugin but maybe its a part of event manager plugin that I have instaled …
    but I resolved it with deactivate and reactivation of the plugin
    from that point everything works perfect
