[Plugin: Usernoise modal feedback / contact form] Modal window does not show up

Hi,
I have successfully installed and activated usernoise, and done required settings, the feedback button shows up but the modal window does not open.
I am using version 0.5.1. and i am using channel theme by theme-junkie and also featured content gallery plugin.
