[Plugin: Uploads Folder] Nothing happens

After the plugin, nothing happens when I’m uploading new media (photos in this case). The files are uploaded correctly, but in the original folders, not under the date/post name folder that this plugin is supposed to do. No error messages or warnings are generated in the server logs.
