Plugin to report video issues?

I’ve visited some other wordpress sites that have something implemented to report when a video isn’t working. I’m looking for something similar for a youtube kinda site i’m running, would certainly make my life easier if I could give my users this option and have accurate reporting on what posts are broken etc.
