Plugin to add form result to page

Hello,

I would like to do the following:
– On a page, a visitor can fill in text fields, images, tags… similar to forms
– The issue of the form is added and displayed on a second page in a table with all different fields, like in a gallery in a way. The last entry is at the top. Each page displays something like 10 entries and the older entries are in the following pages. (as a blog can do). The principle is more or less the same as guestbook or the worpdress comments unless the use is not the same.

– Does plugin exist, free if possible to do this ? Is it a good idea to imagine populatinhg a database as intermediate step ?

– I have already looked around for free forms a bit, but I haven’t found any to do this.

Any idea ?
