    even when there is no content on the page, the formatting is messed up

    I have tried this install on a couple of different WordPress sites with different themes.

    The template was originally created based on the default theme prior to version 2.8. You can create a custom template based on your default one, but make sure you name it the same way as the template that comes with the plugin (copy the meta data from the top of the original template into your custom one). When you assign ThickBox template to a page, that page will appear in the selection list within the ThickBox Content widget. Then you can link that page from a different page, by clicking on the ThickBox Content icon in the page editor, and selecting a page form “Select ThickBox Content” selection list. Hope this makes sense.

    Just FIY, the thickbox template is a basic page template, but without a header, footer and sidebars just because we only need the content part in it.
