[Plugin – Theme My Login] Errors with IIS6.0

We’ve been developing a private area for users only. We’re using the plugin Theme My Login (https://www.jfarthing.com/docs/category/theme-my-login) to handle user registrations but for some reason it’s having the following problems when we put it on the production server:

– Shows login form on the lost password page (https://www.nbrii.com/login/?action=lostpassword) and register page (https://www.nbrii.com/login/?action=register)

– When you logged out, it either sends you into a redirect loop or tells you you’re already logged in.

We’re unable to upgrade the server or move it. We’ve disabled the plugins, switched themes, and removed any customization on the plugin. Nothing works.

Any ideas?
