[Plugin: TDO Mini Forms] Need a lot of help!!

    I am quite new to the world of WordPress and feel I’m a bit out my depth here!

    I’m trying to set up this website so that registered users can easily submit reviews.

    I would like them to select:

    A league > Referee
    Home Team
    Away Team
    Date
    Star Ratings
    Text Review if Desired

    Once a review is submitted, I would like users to reply to this, with star ratings optional.

    Can anyone give me advice on how to go about this, or point me in the direction of a guide?

    Also, I would like each main category to appear along the top (rather than each page), but this is really a side issue at the moment.

    Another question: Is is possible to set up a database, so that when a particular referee is selected, the post will include a picture automatically?
