[Plugin: TDO Mini Forms] how to force deleting a form from database ?

i tried to make a form. i was named default-form and stay near home link, in header.

i wanted to change the name of the form. didnt want to. so i unninstalled the Plugin TDO Mini Form. but default form is still there near home link.

i installed plugin again, make a form with my desired name and wanted to delete the old form.

– is not everywhere in admin pannel plugin.
– in not in header php or indexes.

maybe in database ? how to remove that link (/default-form) ??

pls. help me somebody.

wp 2.71. wp super cache, cleared cache.
