[Plugin: TDO Mini Forms] Error after activation

Hi, I cann’t make this plug in work in my blog. I unninstalled all plug ins, activate this one and works. Then after activating any other plg in, it doen’t matter wich one, WP crashes with a 500 internal server error.
Any suggestion? Thanks in advance!
