• Hi – I have been using four surveyme forms on my site with no problems. Recently I added a checkbox question to each of the surveyme forms. After this, one of the required fields went missing on the results form and the following error message was seen on one of the questionnaires when it was submitted (other different error messages appeared on the other forms)

    Invalid query: You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near ‘formnew ( id bigint(20) NOT NULL AUTO_INCREMENT, PRIMARY KEY (id) )’ at line 1 Whole query: CREATE TABLE IF NOT EXISTS wp_surveyme_6th formnew ( id bigint(20) NOT NULL AUTO_INCREMENT, PRIMARY KEY (id) )

    Can you help me to solve the error messages and also to get all of the information to show on my results email.

    The form name contains a space. This is not allowed in the form name as the form name is used as part of the table name.
    Newer versions of the plugin do not allow you to enter space.
    As to why it worked before I am unsure.

    The quickest way for you to fix the problem is to copy the form to a new form.
