[Plugin: SS Quiz] PLugin is not reacting

I also noticed that its redirecting to #ssheader but nothing happens.
Can someone help me please?

Do you have wp_head(); declared in your header.php template file?
If not add this piece of code before </head> tag:

<?php wp_head(); ?>

This will get some jquery and ajax scrips needed for propper runnig this plugin.

I jut checked and i have it in the header.
The funny thing , its was working before and just stopped now.

You have some errors:

TypeError: $m(“.ccf-datepicker”).datepicker is not a function
$m(“.ccf-datepicker”).datepicker();

Try to deactivate the plugin for your contact form and see if works now.
