[Plugin: Simple PopUp] Adding fields

Hi. I would like to have several fields on the popup (Name, Email, Phone and MLS#). Is there a way to do this?

Also, I’d like to have the MLS# field (or post Subject field) populate using the page/post title.

I’m not a coder, but can navigate my way around and appreciate any assistance.
