[Plugin: Sidebar Login] problem with sidebar login

I used this widget for a few weeks and now I no longer need it.
When I deactivate the plugin, my blog no longer loads correctly, it’s just blank. I used the widget, never added the code manually. I have made NO code references to this anywhere in my template.
