    I’m 99% sure that the second error is the one that caused the 500, but it would have only happened if permissions on the /wp-content/plugins/ directory were set to 777. Can you check the permissions on that directory? If they’re set to 777, set them to 755.

    The permissions were set at 777 and I set them to 755.

    I tried the test again and I get the same Internal Server Error message.

    maybe other folders are set to 777 and should be 755
    Check all these:

    /wp-content
    /wp-content/plugins
    /wp-content/plugins/si-captcha-for-wordpress
    /wp-content/plugins/si-captcha-for-wordpress/captcha-secureimage

    The wp-content was set to 777.
    I set it to 755 and the CAPTCHA works perfectly.
