Plugin set up and config working but cannot see buttons in Visual or Text Editor

I’ve actually gone through each folder and file associated with the plugin and set the permissions to 777, still I can’t see the buttons on visual or text editor mode.
Furthermore I can’t locate the plugin icons through raw source code on my browser.
Any idea what’s going on here. Great plugin though.
