[Plugin: Secondary HTML Content] Feature Request: Naming Fields

It would be fantastic if we could name the fields 1-5 so that it is obvious what each extra box is for.

It would also be great if we show these extra boxes to only pages marked with a specific template.

I feel these two features would really make this a stellar plugin.
