plugin only working on home page

I am trying to use the plugin WP Customer Reviews but it only works on my home page. You can see the ‘Click here to submit your review’ button on the bottom of the page at https://drivinglessonseastbourne.com/ I am currently using the Canvas theme. In fact the plugin works when I save the page as a draft but as soon as I publish the page it disappears.Can anybody help with this issue – thanks
