As you opened a topic here and created a review 5 minutes later saying the plugin doesn’t work, I imagine you found an alternative, so if you don’t mind, please share it here with the community. (This plugin is a side project for me, so I’d love it if people could rely on a more sustainable project.)

I also saw you created 7 plugin reviews (including this one) here at WP.org, all of them complaining about something. It’d be great if you could raise some praise for the plugins that do work for you! ??

it works on version 5.8, i have form submit error on submit button click. Fix it by using [your-email__1] on Mail tab, although it gives an error about incorrect syntax.
