[Plugin: NextGEN Public Uploader] Additional Form Fields?

I’m interested in using the plugin with the NextGEN Gallery, but wanted to know if I can add more form fields to the Image Uploader. This could potentially be a dealbreaker so feedback is greatly appreciated.
