[Plugin: MailChimp List Subscribe Form] One field of Mail Chimp form does not embed

I added the mail chimp form to a blog and one of the fields does not show up. The form I set up in mail chimp includes a radio grouping “how did you hear about us” and then a drop down list, but it does not show up on the blog:
