[Plugin: Login With Ajax] clash between widget and shortcode

if you have the widget on the sidebar and then add a shortcode to have the login panel show up in your page it doesn’t send the correct login data

reason is that the plugin is does not set up different names for the inputs so when you hit login on the page shortcode generated login panel the plugin looks at the widget inputs which will of course normally be empty
